Electronic-only Statement Delivery

The Electronic-only Statement Delivery service (also known as E-Statements) sends your monthly account summaries, balances, and disclosures to your online account in electronic-only format through our secure website.

Your electronic statements are exactly the same as your paper statements. Each month you will receive an email informing you when your statement is available online. Your statement will remain online 24 months on our secure web site and may be downloaded or printed for permanent retention.

To begin the process, we require your consent for the service so that we can send your statements in electronic-only formant and suspend sending your paper statements in the mail. To access your account statement online, you will also need to meet the following minimum hardware and software requirements.

• A computer with internet connection and a hard drive with sufficient hard disk space to save any statements.
• A printer connected to your PC to print statements, (if you desire a hard copy for your records).
• An Internet browser with 128-bit encryption. (Internet Explorer 4.01, Firefox 1.5 or later, and AOL 4.0 or later to support this level of encryption) and Internet access through an Internet Service Provider (ISP).
• A home email address.

You will be notified if these requirements change. If you select Electronic-only Statements Delivery but change your mind at a later time, you can opt-out of electronic-only statements and receive paper statements from Farmers State Bank at the next statement cycle.

Free Bill Pay Option

The Internet Banking Bill Pay service fee ($4.95) will be waived if you elect to receive the Electronic-only Statements.

IMPORTANT INFORMATION ABOUT PROCEDURES FOR OPENING A NEW ACCOUNT

To help the government fight the funding of terrorism and money laundering activities, Federal law requires all financial institutions to obtain, verify, and record information that identifies each person who opens an account.

What this means for you: When you open an account, we will ask for your name, address, date of birth, and other information that will allow us to identify you. We may also ask to see your driver’s license or other identifying documents.
